Understanding and Delivering Person-Centred Support.

This CPD-accredited awareness course gives care workers and support workers a clear, practical understanding of care planning and what effective, person-centred care planning looks like in UK care settings.

Learners will explore the purpose of care plans, how legislation and capacity frameworks influence practice across the UK, and how to use care plans in day-to-day support to promote dignity, safety, consistency and independence.

  • Identify the key components of a care plan, including goals, risks, interventions and review arrangements.

  • Set and support SMART goals that reflect what matters to the individual.
  • Communicate effectively, record factual observations, and understand when care plans should be reviewed or escalated.

Overview

This online training course has been designed specifically for Care and Support Workers working in a social care setting. It may also be useful to a wider audience of people who would like to know more about Care Planning

  • What a care plan is and why it is central to safe, coordinated care
  • The core components of a care plan (needs, goals, interventions, risks, communication, mobility, nutrition and review processes)
  • Person-centred care and involving individuals in planning and decision-making
  • UK legal frameworks that influence care planning, including capacity legislation and safeguarding duties
  • Goal setting using the SMART approach
  • Effective communication in care planning conversations
  • The importance of regular review and keeping care plans accurate and up to date
  • Digital care planning systems and secure record keeping
  • Practical scenarios demonstrating good and poor communication
  • Short narrated videos, interactive tasks (sorting, matching, case scenarios) and realistic care examples
  • Final knowledge check and CPD-accredited certificate (pass mark 80%)

Approx. 40 to 60 minutes, including interactive tasks, video scenarios and knowledge checks.
